The John Jay Soccer Program requires various forms for activities throughout the year, including Winter and Summer Soccer, Fall Away game transportation, and Senior Night. Fall Season Forms
≡ ArbiterSports Fall Sports Online Registration
ArbiterSports is the online platform used for managing player registration, payments, and required forms for John Jay Soccer. All players must complete their registration through ArbiterSports to be eligible for participation. This system ensures a streamlined process for submitting medical forms, waivers, and other essential documents
≡ Transportation Waiver & Release Form
The Transportation Waiver & Release Google Form is required for any John Jay Soccer player who may need alternative transportation to or from games, practices, or team events. This form ensures that parents/guardians acknowledge and approve any non-school transportation arrangements. All players must have a completed waiver on file before utilizing alternate travel options.
≡ Senior Night Questionnaire
The Senior Night Questionnaire Google Form is an important step in preparing for our annual Senior Night celebration. This form allows senior players to provide key details, including their favorite soccer memories, future plans, and acknowledgments for family, coaches, and teammates. The information collected will be used during the event to honor and recognize our graduating players.
Offseason Forms
≡ Winter Soccer Program Registration Form
The John Jay Winter Soccer Program is designed for dedicated players who want to maintain and elevate their fitness and skills during the offseason. This is a competitive training program—not recreational play—focused on preparing athletes for next year’s school season. Each session includes one hour of high-intensity, small-sided games that emphasize possession-based play, technical ability, and quick decision-making. Players will also take part in strength training with Coach Axelson, either before or after their game session, to build endurance, strength, and injury resilience.
≡ Summer Soccer Program Registration Forms
The John Jay Summer Soccer Program is designed for rising 10th through 12th graders who are committed to competing at the Varsity level. This is not a recreational program—it is an opportunity for serious players to develop their skills, fitness, and tactical understanding through structured Summer Training and competitive Varsity-level Summer League matches. If your goal is simply to "play in some games," this program is not for you. Its purpose is clear: to prepare and elevate players for the upcoming high school fall season.
To participate, players must both register via Google Form and print and complete the two required league forms before submitting them.

Middle School Forms
≡ Athletic Placement Process (APP)
7th and 8th graders looking to try out for John Jay JV or Varsity Soccer must complete the Athletic Placement Process (APP). This ensures players meet the physical and developmental standards for high school competition. Print and complete the required APP forms below, including the Maturity Assessment, and submit them on time to be eligible.
